Patients are seen by appointment only at our offices. Our automated calling system will call to confirm appointments two days in advance. Cancellation should be made 24-hours prior to the appointment. A fee may be charged for a missed appointment without proper notification. All children under the age of 18 must be accompanied by a parent or guardian. We ask that patients bring a list of any medications they are currently taking.
We handle patients' personal information in a confidential manner. This information is required from all of our patients to be used for insurance and billing purposes. Additionally, it is required by a lab if a biopsy or other procedure is performed. We are HIPAA-compliant, and all information is confidential.
We currently participate with several insurance companies, including Blue Cross Blue Shield, Medicare with Secondary Insurance, United Healthcare and Tricare. Insurance is a contract between you and your insurance company and it is your responsibility to make sure that our providers participate with your insurance company. All insurance deductibles and co-pays will be collected at the time of service. If we are non-participating with your insurance plan, you will be responsible for payment in full at the time of service.
Our office will provide you with the necessary claim form to submit to your insurance company for reimbursement. You will need to bring your insurance card, driver's license and any co-pay or deductible that will you be responsible for at the time of service. Additionally, all cosmetic and laser treatments and procedures are not covered by insurance and must be paid in full at the time of service. For your convenience, we accept cash, check, and most credit cards.
